Patient Information

Please arrive at least 15 minutes before your scheduled appointment time. 

Please bring the following items with you:

• Referral from your Pediatrician/Primary Care Physician.

• Name, address, phone, and fax number of your Primary Care/Referring Physician.

• Name, address, phone, and fax number of your Pharmacy.

• Copies of lab work, x-rays, MRI scans, etc.  (If you do not have copies of the lab work/scans, please let us know and we will obtain copies on your behalf).

• All medications that are being taken.

• Your original health insurance card.

If there are behavioral/developmental issues, please also bring:

• A baby book with developmental milestones.
• Teacher reports from as many years as possible, including early elementary school.
• A written note from current teachers describing their observations.
• Copies (if any) of psychological or educational testing.

If unusual episodes are an issue:

• Video taping the event is very important.
• Also, observe whether the child can be brought out the of the episode by vigorously trying to get his attention.

Emergencies
Our physicians are on-call 24 hours a day to offer help to patients in need. If you have an urgent problem, after hours or on a holiday, please call our After Hours Service at (316) 686-6866. If this is a true medical emergency, please call 911.

Cancellation Policy
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.
